Here's the information: 

Seen on 6/21/05 at 1:30 PM. in American Fork Canyon; west of Tibble Fork, southwest of Granite Flats.
Descriptions: 6-8 inches tall, sitting in Mountain Maple, very tame. Yellowish gray bill.  Couldn't see distinct markings on back of head.  Tail was only 1 inch long.
